Speech Pathologist/Audiologist

Overview

In this part-time role, you will deliver specialized speech pathology and audiology services directly to patients within their home environments across the near west and south Indianapolis areas. You will conduct comprehensive clinical evaluations, including OASIS assessments, design customized care plans in alignment with physician orders, and educate patients and their families on ongoing therapeutic strategies. You will operate as an integral member of an interdisciplinary home healthcare team, coordinating closely with nursing staff and referring physicians to adjust treatments as patient conditions evolve. This position provides an opportunity to independently manage your patient caseload while ensuring rigorous adherence to clinical documentation standards and quality care guidelines.

Responsibilities

• Conduct initial and ongoing comprehensive clinical assessments of patients' speech, language, and hearing functions, including OASIS assessments.
• Develop, implement, and revise individualized speech therapy treatment plans in direct consultation with physicians.
• Deliver skilled home-based speech pathology interventions and adjust therapies based on patient progress and medical status.
• Educate, counsel, and guide patients and their family caregivers on therapeutic home programs and available community resources.
• Coordinate care plans and communicate regularly with physicians, nurses, and other interdisciplinary team members.
• Maintain timely and accurate clinical progress notes, evaluation data, and discharge summaries in compliance with agency standards.
• Supervise home health services in accordance with agency policy when speech therapy is the primary discipline involved.

Qualifications

• Master's degree in Speech-Language Pathology.
• Active and unrestricted Speech-Language Pathologist license in the State of Indiana.
• Current CPR certification.
• Successful completion of a criminal background check.
• Valid driver's license and reliable transportation to travel to patient homes.
• Physical ability to lift up to 30 pounds, assist with patient mobility up to 250 pounds, and navigate varied residential environments.

Nice to have

• Active membership and certification through the American Speech-Language-Hearing Association (ASHA).
• Undergraduate degree in the field of Speech and Hearing Therapy.
• At least one year of clinical experience within an institutional or home health setting.

Work environment

• Field-based role delivering care on-site within patient residences across near west and south Indianapolis, Indiana.
• Requires regular local travel to access residential homes that may not be routinely wheelchair accessible.
• Part-time schedule with visit coordination confirmed weekly alongside nursing leadership.
