As the Registered Nurse for Adult Services, you will play a critical role in providing clinical oversight and nursing assessments for adults with intellectual and developmental disabilities. You will manage healthcare plans across various settings, including residential and day programs, ensuring that each individual receives high-quality medical coordination and support to live a healthy, independent life.
Responsibilities
- Complete comprehensive nursing assessments and develop individualized health care plans for adults with disabilities.
- Coordinate medical care with physicians, specialists, pharmacies, and interdisciplinary teams to ensure holistic support.
- Oversee medication administration systems and ensure full compliance with QMAP requirements and agency policies.
- Develop and deliver specialized health care training and competency evaluations for Direct Support Professionals.
- Conduct routine health monitoring visits within residential settings to track chronic conditions and follow-up care.
- Perform nursing delegation tasks in strict accordance with the Colorado Nurse Practice Act and Board of Nursing Rules.
- Maintain accurate electronic health records and participate in a nursing on-call rotation as assigned.
Qualifications
- You must hold a current Colorado Registered Nurse license in good standing.
- You need at least two years of professional nursing experience.
- You must possess a valid Colorado driver's license with a reliable vehicle and an acceptable driving record.
- You are required to have a current CPR/BLS certification.
- You must demonstrate proficiency with electronic health documentation systems and professional communication.
- You must be able to pass all required background screenings for healthcare providers.
Nice to have
- Prior experience working with individuals who have intellectual and developmental disabilities is highly preferred.
- Familiarity with Medicaid Home and Community-Based Services (HCBS) or waiver regulations is a significant advantage.
- Experience in community health, long-term care, or case management settings will help you excel in this role.
- Previous experience training and supervising unlicensed direct care staff is beneficial.
Work environment
- This is an on-site position based in Canon City, Colorado, serving the surrounding local area.
- You will work in a variety of settings, including residential homes, day program centers, and community environments.
- The role requires local travel to different service sites, for which mileage reimbursement is provided.
- You will be part of a collaborative interdisciplinary team that includes therapists, guardians, and program leadership.
