Registered Nurse (Home Health)

Overview

In this role, you will deliver comprehensive skilled nursing care and conduct health and psychosocial evaluations for patients in their homes across the west and south Indianapolis areas. You will manage total patient care from admission through discharge, performing standardized OASIS assessments, updating care plans, and coordinating with physicians and multidisciplinary healthcare professionals.

Responsibilities

• Conduct comprehensive health and psychosocial assessments, including admission, transfer, recertification, and discharge OASIS documentation.
• Develop, implement, and periodically revise individualized nursing care plans in consultation with attending physicians.
• Deliver skilled nursing treatments, administer medications, and initiate preventative and rehabilitative nursing procedures.
• Supervise and evaluate Home Health Aides and Licensed Practical Nurses, preparing their care plans and conducting regular reviews.
• Educate and counsel patients and family caregivers on disease management, care techniques, and safety practices.
