Pediatric Homecare | Licensed Practical Nurse (LPN)

Overview

In this role, you will deliver dedicated one-on-one nursing care to infants, children, and young adults with complex medical needs directly within their home environments. You will assess physical and psychosocial parameters, carry out individualized care plans, and deliver skilled clinical interventions while building close, supportive relationships with pediatric clients and their families.

Responsibilities

• Perform comprehensive physical, psychosocial, and environmental assessments of pediatric clients, including OASIS data sets when required.
• Develop, implement, and adjust individualized nursing care plans tailored to each client's specific clinical needs.
• Provide skilled nursing interventions focused on achieving realistic health outcomes within targeted timeframes.
• Communicate significant changes in client condition promptly to physicians and collaborating interdisciplinary care team members.
• Maintain detailed, systematic client medical records covering assessments, care interventions, and patient progress.
