Responsibilities
Conduct comprehensive, 45-minute in-home health evaluations and physical assessments for health plan members.
Review patient medical histories and perform thorough medication reconciliations.
Administer indicated preventive and diagnostic screenings, such as diabetic eye exams, spirometry, and bone density tests.
Document clinical findings and patient observations efficiently using a tablet-based workflow.
Identify home safety risks and social determinants of health to facilitate referrals back to primary care providers and health plans.
Conduct virtual clinical evaluations when required by operational needs.
Travel locally within designated coverage territories to meet patients directly in their residences.
Qualifications
Active and unrestricted Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) state license.
Ability and willingness to cover primary locations across Berks, Bucks, Chester, Delaware, Montgomery, or Philadelphia counties.
Comfort performing clinical evaluations within diverse residential settings that may include pets, stairs, or airborne allergens.
Strong clinical assessment and interpersonal communication skills to engage patients effectively in their homes.
Active, unrestricted medical licenses in multiple states.
Prior clinical experience in home health, primary care, geriatrics, or community medicine.
Familiarity with electronic clinical workflow tools and mobile health documentation applications.
Work Environment
Field-based role with primary work conducted inside the private residences of health plan members.
Full-time schedule requiring approximately 40 hours per week.
Regular local travel throughout Philadelphia and surrounding regional counties.
Occasional work-from-home responsibilities when conducting virtual visits or completing documentation.
