Responsibilities

Conduct comprehensive 45-minute in-home health evaluations for health plan members to assess overall health and bridge gaps in care.

Perform detailed medication reviews, medical history assessments, and physical evaluations during each home visit.

Administer indicated preventive diagnostic screenings, including spirometry, diabetic eye exams, and bone density tests.

Document member encounters and clinical findings using an iPad-based workflow system.

Deliver personalized health education and guidance to empower members in managing their health conditions.

Conduct virtual health visits and evaluations as needed to support coverage requirements.

Travel locally within designated coverage areas to meet patients at their private residences.

Qualifications

Hold an active, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in the designated state of practice.

Maintain national Board Certification relevant to your clinician scope.

Possess the physical ability and comfort to deliver care inside diverse private residential environments.

Demonstrate proficiency with digital clinical documentation tools and mobile health workflows.

Nice to Have

Possess active, multi-state clinician licenses across neighboring jurisdictions.

Bring prior clinical experience in home healthcare, geriatrics, or primary care settings.

Demonstrate strong patient engagement, relationship-building, and communication skills.

Work Environment

Work primarily in the field visiting private residences of health plan members throughout the Philadelphia area, with occasional virtual visits.

Operate within diverse home settings that may include household pets, secondhand smoke, and common environmental allergens.

Maintain a part-time schedule averaging approximately 29.5 hours per week.

Travel locally within assigned geographical territories and licensed coverage areas.
