Pediatric Homecare | Registered Nurse (RN)

Overview

In this role, you will provide specialized nursing care to infants, children, and young adults with complex medical needs within the comfort of their own homes. You will act as a vital link between families and the medical community, ensuring that your patients receive high-quality, individualized attention while supporting their families in managing intricate healthcare requirements. By working independently outside of a traditional hospital setting, you will have the unique opportunity to build deep, one-on-one relationships with your clients and participate directly in their daily therapies and community engagement.

Responsibilities

• Perform comprehensive physical, psychosocial, and environmental assessments of pediatric clients in their home settings.
• Develop and implement individualized nursing care plans that address the specific medical and developmental needs of each child.
• Provide skilled nursing interventions such as tracheostomy care and ventilator management to achieve positive health outcomes.
• Report significant changes in client status to physicians and other members of the multidisciplinary medical team.
• Maintain accurate and systematic clinical documentation, including OASIS data sets and progress reports.
• Coordinate with families and caregivers to facilitate the client's participation in external therapies and community activities.
• Modify and update care plans regularly to reflect the patient's progress toward established health goals.

Qualifications

• You must hold a current and active Registered Nurse (RN) license in the state of Indiana.
• You are required to have a valid driver's license and maintain active auto insurance.
• You must have access to reliable transportation to travel to clients' homes in the surrounding regions.
• You must demonstrate the ability to work independently and manage complex patient care without direct on-site supervision.
• You must be willing to complete a comprehensive background check as part of the hiring process.

Nice to have

• Previous professional experience managing tracheostomy and ventilator-dependent patients is highly preferred.
• Experience in pediatric nursing or home health care environments is beneficial.
• Familiarity with OASIS documentation and home health regulatory standards is an advantage.

Work environment

• This is an on-site, home-based position requiring travel to clients in Warsaw, Goshen, Pierceton, and Columbia City.
• The role offers flexible scheduling options including full-time, part-time, and weekend-specific shifts.
• You will work in a one-on-one care environment, providing a more intimate and focused setting than a traditional hospital.
• The position allows you to accompany clients outside for community activities and therapy sessions.
