Speech Pathologist/Audiologist

Overview

In this role, you will provide essential speech pathology and audiology services to patients in their homes across the west and south Indianapolis areas. You will take ownership of the full clinical cycle, from conducting initial OASIS assessments and developing customized treatment plans to coordinating with physicians and interdisciplinary teams to ensure high-quality patient outcomes.

Responsibilities

• Conduct comprehensive initial and ongoing assessments of patient needs using OASIS assessment tools.
• Develop and implement individualized speech therapy treatment plans in consultation with attending physicians.
• Educate and counsel patients and their families on therapy programs and home-based care techniques.
• Evaluate the patient's home environment to determine necessary community resources and instructional requirements.
• Collaborate with interdisciplinary care teams to ensure seamless delivery of healthcare services.
• Maintain accurate clinical documentation and progress notes within required regulatory timeframes.
