In this role, you will provide critical one-on-one nursing care to infants, children, and young adults with complex medical needs within the comfort of their own homes. You will act as a vital support system for families, helping them manage sophisticated healthcare requirements while maintaining the independence and dignity of your young patients. Working away from the constraints of traditional hospital settings, you will have the autonomy to facilitate daily activities and therapies that help each child reach their full potential.
Responsibilities
- Perform comprehensive physical, psychosocial, and environmental assessments of each pediatric client.
- Develop and implement individualized nursing care plans tailored to the specific medical needs of the child.
- Provide skilled clinical interventions to achieve realistic health outcomes within established timeframes.
- Coordinate with physicians and multidisciplinary team members to report status changes and update care strategies.
- Maintain systematic and accurate client records covering assessment, planning, and evaluation phases.
- Assist clients in participating in external therapies and community activities to improve their quality of life.
- Collect and document OASIS data sets to ensure regulatory compliance and high standards of care.
Qualifications
- You must hold an active Licensed Practical Nurse (LPN) license in the state of Indiana.
- A valid driver's license and proof of auto insurance are required for travel to client homes.
- You must have access to reliable transportation to reach various service locations.
- Successful completion of a professional certificate program in nursing is necessary for this position.
- You must be able to pass a comprehensive background check in accordance with state laws and company policy.
Nice to have
- Prior experience working with tracheostomy care is highly valued.
- Technical proficiency with ventilator management is preferred for higher-acuity cases.
- Previous experience in pediatric home health or a similar one-on-one clinical environment is beneficial.
Work environment
- This is an on-site role providing care directly in the private homes of clients.
- You will serve the Warsaw area, with potential assignments in nearby communities like Goshen, Pierceton, and Columbia City.
- The schedule offers flexibility with part-time weekend shifts and night shift opportunities available.
- You will work independently but maintain regular communication with a remote clinical support team.
- The role involves local travel to reach different client residences within your assigned territory.
