Registered Nurse (Home Health)

Overview

In this role, you will provide comprehensive nursing care and psychosocial evaluations for patients within their homes across the Indianapolis area. You will act as a primary clinical lead, coordinating total patient care through the development of treatment plans, conducting OASIS assessments, and ensuring all services align with physician orders and regulatory standards.

Responsibilities

• Conduct comprehensive health and psychosocial evaluations to monitor patient conditions and promote preventive practices.
• Perform admission, transfer, re-certification, and discharge OASIS assessments for home care patients.
• Coordinate with attending physicians to alter patient care plans and implement necessary change orders.
• Supervise and evaluate the performance of Home Health Aides and Licensed Practical Nurses to ensure high-quality care delivery.
• Maintain detailed clinical notes and progress reports outlining all services rendered on a weekly basis.
