Medical Records Coder
Translating medical procedures and diagnoses into standardized alphanumeric codes for billing and documentation.
Overview
The daily work involves a systematic review of physician notes, lab results, and surgical reports to identify every service provided during a patient encounter. This role requires a high degree of focus as coders navigate electronic health record systems to ensure every diagnosis and procedure is accurately represented by current ICD-10, CPT, and HCPCS codes. The rhythm of the work is consistent and production-based, requiring steady output and precision over long periods of solitary analysis.
Success in this field relies on an analytical mindset and an interest in medical terminology and anatomy. The job feels like solving a technical puzzle where clinical narratives must be reconciled with rigid classification systems. It is well-suited for individuals who find satisfaction in detail-oriented tasks and the implementation of complex regulatory guidelines. While the work is largely independent, coders must occasionally communicate with medical staff to clarify documentation discrepancies.
Responsibilities
- Review clinical documentation to extract relevant diagnoses and procedure information.
- Assign accurate ICD-10-CM, CPT, and HCPCS codes for reimbursement and data analysis.
- Verify that medical records meet all legal and insurance requirements for documentation.
- Collaborate with billing departments to resolve claim denials and coding errors.
